Admin - Managing an ED patient with active Inpatient Encounter (FirstNet and PM)

If a patient with an open inpatient encounter presents to the emergency department (eg. HITH patient/ gate leave patient/  C2N patient who has deteriorated) the original inpatient encounter should not be discharged.

 

  • The inpatient encounter remains open, but is transferred to the virtual “EMERG Inpatient” location using Modify Encounter Conversation 

AND

  • A new Emergency encounter is created via the standard process in FirstNet. This puts the patient on the tracking board. The patient can then be triaged and full registration can occur as normal. 

 

NOTE:   If using eMM, medications must be managed on patient's inpatient visit.   All other documentations and orders are to be managed on patient's emergency visit. 

 

When the patients leaves the Emergency Department, both encounters need to be managed.

  • If the patient is discharged home: 
    • Emergency encounter: Use 'Actual Discharge' to discharge ED encounter 
    • Inpatient encounter: Follow normal process in Access Management Office 

OR

  • If the patient is admitted (to ward or EMU):
    • Emergency encounter: use 'Actual Discharge' to discharge ED encounter 
    • Inpatient encounter: Use transfer conversation within Access Management Office to move patient to inpatient/EMU ward location
