BTF - Increasing the frequency of observations for nurses

BTF Observations must routinely be performed every 4 hours as a minimum for paediatric patients. In the event where observations are being performed more frequently, nurses are to document this change on the BTF Observation Chart.

  • Click BTF Observation Chart from the Menu

  • Click Frequency

  • Select the appropriate Prescribed Frequency of Observations from the drop down:
    every 15 minutes; every 30 minutes; 1 hourly or 2 hourly.

  • Enter the Rationale for the Variation to Frequency of Observations
    This is mandatory and requires a minimum of 20 characters
  • Enter the name of the Authorising Medical Officer (AMO), alternatively click the magnifying glass to search for the AMO or tick ‘Use attending doctor’

  • Click the green tick to sign off the form

  • The set Frequency of Observations will be displayed and the button is displayed blue

  • Any changes to the prescribed frequency of observations will be displayed in the table below

Review the Variation to Frequency of Observations

  • Click the Frequency button

  • Review the History – Prescribed Frequency of Observations table

  • Click Reset to Standard Frequency of Observations to revert to 4 hourly BTF Observations if appropriate

  • Repeat steps listed above to make a Variation to Frequency of Observations
