Advance Care Planning refers to the conversations between patients, families, guardians, and health professionals to plan ahead for potential deterioration.
The ACP product groups together several components that assist clinicians in discussing Advance Care Planning with patients and families, as well as documenting/indicating plan details:

Resuscitation Plan – electronic version of the paper adult & paediatric Resuscitation Plan

Advance Care Planning Documentation – recording whether a patient has documentation including Advance Care Directives and whether a copy has been presented/scanned.

Identify whether a Resuscitation Plan exists for a patient

View a patient's current Resuscitation Plan

View a patient's discontinued Resuscitation Plans from previous admissions

Identify if the patient has any Advance Care Planning documentation in the eMR
