All About Me is aimed to provide staff with tips and advice regarding a child or young person’s communication abilities, behaviours, likes, dislikes, routines and rituals that are gained by the care giver or parent/s.
There are 2 methods for documenting this information in SCHN eMR, which will trigger the All About Me Notification tile:
- All About Me – Top 5 ad hoc form
- All About Me – Hospital Passport clinical note (for import of scanned PDFs completed by parents and carers)
All About Me – Top 5 AdHoc Form
Click on AdHoc
The All About Me – Top 5 form is located under General Forms, or Emergency Department forms
Enter information in fields below – you must enter at least 1 TOP5 tip

Once completed, click on small green arrow in top left corner to Sign the form
The All About Me tile will appear in the Notifications component
Click Show Details to bring up a window to view the TOP5, as well as the Hospital Passport clinical note, and other relevant resources
The pink puzzle icon will also appear in the alerts on ED tracking boards. Open the patient chart to locate the All About Me tile on ED View
All About Me - Hospital Passport Clinical note
To upload a scanned Hospital Passport for a child or young person, complete the following steps:
Click on Clinical Notes
Click on the small scanner icon
For the Type, select All About Me – Hospital Passport
If this note type is not in your list right click on Type field, select Document Type and then Complete – this will show all documents, you can search for All About Me – Hospital Passport)
Click Import at the bottom right of the Clinical Note window
Select your scanned PDF from the network drives eg, K: drive, G: drive etc
Click Open to import it
A preview will be included in the note. You can also add text or comments in the lower half of the note
Click Sign to finalise the document in PowerChart
The link to Hospital Passport PDF will appear in the All About Me tile

This provides the carer/parent to offer valuable information about how to assist with caring for their child with special needs such as Intellectual Disability and/or Autism
Print, Email or Complete online/send/give to parent
