Follow the steps below to complete the Ad hoc PECAT Client Referral form in PowerChart.
1. Open PowerChart
- Click the magnify glass to bring up the Person/Encounter Search window
- Search using multiple positive patient identifiers e.g. full name, sex, D.O.B, medicare no.
- Click Search
- If the patient is found in eMR, highlight an applicable encounter
- Note: Best practice for PECAT would be to select a CHW encounter. This is displayed as the Royal Alexandra Hospital for Children.
- Click OK to open the patient’s chart
- If the patient is not found in the eMR, and if enough information, register patient in Patient Management (PM)
- If not enough information, call the patient for registration information, then register
Note: Follow this QuickStart that covers how to register a patient in PM or refer to your patient registration administrative workflows. Once the patient is registered, refer to the steps above to search for and open a patients chart
- Click on AdHoc in the PowerChart toolbar
- Click on the Outpatients Forms folder
- Click on the PECAT sub folder
- Open the PECAT Client Referral form
- Complete form as necessary including type and date questionnaires sent
- Click the green tick to sign off the form
2. When questionnaires are returned:
- Click on Form Browser in the PowerChart menu
- Find the required form, right click and select Modify
- Information in previously completed fields will be retained
- Update/modify form as required e.g. document questionnaires returned date
Note: Alternatively, you can repeat step 1 to complete a new form and the information in previously completed fields will also be retained.
3. Once all questionnaires are returned and/or the patient is ready to have an Intake appointment scheduled (appointment does not need to be arranged with family at this stage):
- Click Yes in the field Form Completed and ready to schedule an appointment
- Add comments to indicate if the patient will skip intake and go straight to assessment
- Sign form off
- This will place a task on the Multi Patient Task List
