PowerForms - Enter, Modify and Cancel AdHoc Charting

AdHoc charting allows staff to complete forms and assessments directly into the patient record via a PowerForm. 

Locating PowerForms (AdHoc Charting) 

Via AdHoc in the toolbar 

  • Open the patient record and click the AdHoc icon adhocicon in the toolbar 

  • Click the required folder (left) for the area of interest. This opens the related forms and assessments on the right

  • Tick the required form (right) and then click the Chart button (or double click the form)

  • The PowerForm opens

Via Document Launcher 

  • There are quicklinks to the adhoc forms you use most frequently in your Document Launcher on Paediatric Summary and also ContinuousDoc.

Completing PowerForms 

  • Fill in the relevant fields 

  • Sign by clicking tick in the top left corner 

  • The form is viewable in Documentation, ContinuousDoc and Form Browser 

 

Modify or Cancel AdHoc Data 

Details on a form can be modified or the entire form can be uncharted if it was completed in error.  Modify and Unchart are performed in Form Browser 

  • Click Form Browser from the menu of the patient’s chart fb

  • If required, use Sort By: to find the form more readily e.g. by date or form 

date
  • Right click on the form to be modified or canceled

To Modify: 

  • Right click and select Modify to re-open the form 

  • Complete modifications 

  • Sign tick

 

To Cancel 

  • Right click and select Unchart 

  • Enter a reason for cancelling the form (e.g. wrong patient) 

  • Sign tick
  • Form Browser will now display the form with the updated status of Modified or In Error.

Tips on Completing PowerForms

  • Yellow fields are mandatory yellow

  • Circle options circle: Only one option can be selected  

  • Square options square: more than one option can be selected

  • Some forms have multiple pages listed on the left. View the page titles more fully by dragging the bar to the right

  • Pages with aster have yellow mandatory fields that must be completed
  • As pages are completed, a t appears next to the page title

  • Pages may be inactive (greyed out) and become available depending on answers provided 

  • Move between the pages by clicking the page title 

  • When a response opens another page, return to the original page by clicking ba the in top left

  • Sign the form when all pages are completed. You do not need to sign each page separately 

 

Tools

  • cancel      cancel charting the entire form
  • clear     clear the page entries

  • calc     clinical calculator

    • select the clinical formula from the dropdown and use keypad 

    • click calc button at the bottom left of clinical calculator for a regular calculator

  • arrow up to move up or down the page menu
  • Clear a field by right clicking over it and select Clear
