Procedure – VAN Service Contact Form
- Click the Adhoc button on the PowerChart Toolbar
- Click the Child Protection Folder and select the VAN Service Contact Form
- Click Chart
The VAN Service Event Form has 3 Sections:
- Service Contact
- VAN
- VAN Medical Forensic.
The red asterisk next to the section name indicates mandatory sections that must be completed. The form is unable to be signed as complete until all the mandatory fields have been completed.
Service Contact Section
Service Event Core Data Elements
- The fields on the Service Contact section are enabled depending on which value is selected in the Reporting Type field.
- Depending on the option selected in the Reporting Type field it will automatically populate the Service Delivery Mode and Session Type field.
If Inpatient is selected in the Reporting Type field
The following fields will auto-populate:
- Setting Type
- Admitted/Emergency Service Contact
- Provider on/off campus
If ‘Student’ is selected in the Reporting Type field
Only the following fields will be enabled:
- Indirect Times
- Clinician field
- Clinical Area
- Interventions
- Clinical Findings
- Local Flags
Allied Health/VAN Data Elements
- Clinical Area defaults to ‘Violence, Abuse & Neglect’ on this form. This selection can be changed if required.
- You can enter up to 3 interventions and clinical findings on the form.
- There is conditional logic built against the intervention fields. If “Medical examination” or “Medical forensic examination” is selected in any one of these fields the VAN Medical/Medical Forensic Details section will pop up.
- If the time between Medical/Forensic Exam Request Date/Time and Medical/Forensic Start Date/Time is greater than 2 hours, the Main Reason for Medical/Forensic Exam Delay field becomes mandatory and a reason/s must be entered.
- Click the circular arrow to save this section and go back to the Service Contact Section.
Once the VAN Medical/Medical Forensic Details section is completed, move to the Violence Abuse and Neglect Section and complete the relevant fields.
Violence, Abuse and Neglect Section
This section lists some of the VAN Data Set Extension data elements. This section can be completed for all service contacts, but the section will become mandatory if the value of are following are selected in any of the intervention fields:
- Crisis Management/Support,
- Medical Examination or
- Medical Forensic examination
- Condition logic is built against the fields: “Crisis support Request Date/Time”, “Is the Most Recent Assault/Abuse Dt/Tm Estimated?”, “Most Recent Assault Abuse Date/Time” and “Most Recent Assault/Abuse Time Period”.
- Crisis support Request Date/Time is only enabled when a value of Crisis management/ support is selected in any of the intervention fields.
- Most Recent Assault Abuse Date/Time is only enabled when the value “No” is selected for Is the Most Recent Assault/Abuse Dt/Tm Estimated?
- Most Recent Assault/Abuse Time Period is only enabled when the value “Yes” is selected for Is the Most Recent Assault/Abuse Dt/Tm Estimated?
- When all sections of the form are complete, click the green tick to sign off the form.
To view, modify or unchart the form:
- Click the Forms option in the table of contents
- Right click on the relevant form to reveal the drop-down menu
- Select either View, Modify or Unchart
