How to complete the MH Contact Form

Step 1: Open the patient’s chart in the correct encounter
- It is critical to select the correct encounter for Activity Based Funding!
- Check the location in the top right of the patient record
- Check the admission date - a current inpatient will have No Discharge Date
- If it is the wrong encounter, change the encounter by clicking on the location link and selecting the correct encounter from the visit list

Westmead Outpatient - lower case
