What is the MH Client Contact Form used for?

Mental Health Clinicians must record all clinical activity with a client.  These may be direct or indirect activities - by any method at any location.

How to complete the MH Contact Form

Step 1: Open the patient’s chart in the correct encounter

  • It is critical to select the correct encounter for Activity Based Funding!
  • Check the location in the top right of the patient record
  • Check the admission date - a current inpatient will have No Discharge Date
  • If it is the wrong encounter, change the encounter by clicking on the location link and selecting the correct encounter from the visit list
