Medication Manager - Pharmacy Clinical Interventions

A Clinical Intervention is used to document clinical activities undertaken either for individual medication orders or for the patient. 

Interventions added for a specific medication order

  • Click Medication Manager

  • PharmNet Medication Manager opens
  • Select Patient

  • Select Acute Profile tab

  • Navigate to the medication requiring intervention
  • Select Intervene from the drop down Action menu 
  • Click Apply

  • The Clincial Pharmacy Ad Hoc Charting Folder opens

  • Select the Pharmacy Clinical Interventions form

  • Click Chart

  • The Pharmacy Clinical Interventions form opens

Interventions added for the patient – Medication Manager

  • In Medication Manager, navigate to the bottom left of the screen.
  • Click on the blue + symbol under Rx Interventions 
  • The Clincial Pharmacy Ad Hoc Charting Folder opens as per interventions added for a specific medication order

Interventions added for the patient – PowerChart

 

  • In Powerchart, navigate to the AdHoc button

  • Click on the Clincial Pharmacy Folder
  • Select the Pharmacy Clinical Interventions form and click Chart

  • The Pharmacy Clinical Interventions form opens

Completing the Form

  • Complete all relevant & mandatory fields
  • Multiple medications can be added to the Medications field if they are all affected by the same issue (e.g. counselling required)
  • If multiple issues apply to one or a group of medications, you can select as many Issues Identified as you need
  • If you have more than one kind of issue to log for different medications, please complete separate intervention forms for each medication

  • To see definitions for High Risk Medications, right-click in this box and select reference text

  • The reference text links to relevant policies from the CEC and NSW Health

  • The Summary of Issues section is mandatory and will appear in the progress notes
  • If you require more space to document recommendations, you can also add more detail to the Recommendation section which will appear in the notes under a separate heading.
  • The Action/Outcome section has been reserved for if you need to document an addendum to the form after signing it (e.g. if the recommendation is not acted on, or an adverse event is observed, or extra information is discovered after counselling the patient etc.) See below for more detail

  • If a doctor has been contacted, please search for them by name and enter it in Prescriber Contacted 
  • Please log the IIMS number if known (or yes if not known) and the time taken to complete the intervention (including any research or patient counselling time) to assist the Medication Safety Pharmacist when reviewing and analysing the interventions

  • To sign the form, click the green tick

Note: DO NOT click the save icon, you must SIGN the form for other clinicians to see the document

  • The intervention can now be viewed by everyone via Continuous Doc in PowerChart

Modifying/Updating an intervention

  • To modify the form, go to Form Browser in Powerchart

  • Right click the form you want to update and select Modify

  • Add or update any missing information

  • The additional details will append to the top of the original document in Continuous Doc

  • Modifying the original details of the form will place large sections of strike-through in the text. This is why the Actions/Outcome section has been designed to give you an extra section to update the intervention without this occurring
