Pharmacy - Admission Note

For comprehensive medication management planning and documentation for complex patients, you may wish to use the Pharmacy Admission workflow to help create the Pharmacy Admission Note. This tool has been designed to direct you to different sections of PowerChart in a structured way to help prepare the information in the correct places in PowerChart, then pull them all together when creating the document (similar to how medical discharge summaries are created).

1. On the Pharmacy Summary page, click on Pharmacy Admission

2. The Pharmacy Admission workflow will pull through information from different parts of PowerChart, some for review, others to include in the final documented note

3. Workflow Sections 

a. Allergies

  •  To add or update allergy information, click on the + icon to the right of the screen to open the pop-up allergy documentation tool

  • Clicking on the Allergy heading will take you here:

  • Return to the summary page and click the section refresh icon to update the information

  • The allergies will update
  • If there are no changes to be made, click on Complete Reconciliation to acknowledge the information is correct

b. Problems and Diagnoses

  • Any diagnosis information documented for the patient can be reviewed here. This information is not generally completed by pharmacists and has been included for review purposes

c. Home Medications 

i. For new patients, click on Complete history to document the patient’s medication history. The Medication History documentation screen will open

ii. For returning patients, the medication history information can be reviewed directly from this list. Prescriptions will include estimated supply details. The Cancel/DC and Complete actions can be performed directly from this screen by clicking on the medication name

iii. The Cancel and Complete actions can be performed directly from this screen by clicking on the medication name. Please note Renew is a prescribing action and this will generate a prescription. Do not click Renew for prescription medications

  • The prescription icon appears differently on this screen, it is a blue icon with a red and blue capsule instead of an orange pill bottle. The medication history icon is the same as on other screens (white scroll).

  • To complete the medication history for these patients, you will still need to open the Medication History Tool by clicking on Complete History and removing the tick from Leave Med History Incomplete – Finish Later and then click Document History

  • Included for information only, but new results can be documented from this screen by clicking on the + button in the top right of the section

4. The Pharmacy Admission workflow page does not need to be saved, it can be updated at any time until you are ready to create the document.

5. Once the review has been complete, you are ready to create the Pharmacy Admission Note.

6. Navigate to the Create Note option at the bottom of the workflow menu and select Pharmacy Admission Note

7. The Pharmacy Admission Note template will open automatically, pulling in information you have reviewed in the workflow page. There are also a number of optional extra headings where you can enter free-text information, or choose to remove if they are not relevant for the patient

8. To update any sections, hover underneath the heading and click in the box that appears. The cursor will appear allowing you to type

9. If you notice something needs to be updated (e.g. a missing home medication) you can update the detail in the home medication tool then refresh the section in the note. Click the refresh button that appears next to the section heading when you hover over the area to the right

10. Some sections can be removed if they are not relevant for the patient by clicking on the X that also appears when you hover next to the section heading

11. You can choose to Save the note if you need to come back to it later (will not appear in the medical record) or click Sign/Submit when it is complete

12.  After signing, the Pharmacy Admission Note will be available to search in Continuous Doc
