My Health Memory by Cartula
The My Health Memory tab is available in the Admission, Check-In, Modify and My Health Memory Account Holder Conversations in Patient Management
To create an initial My Health Memory Account Holder for the patient:

Open the patient's chart in Patient Management

Click on the My Health Memory Tab
If the Account Holder already has a relationship established with the patient, they will appear in the "Relationship" pop-up

Select the Account Holder's Relationship to person

The Account Holder's details will be transferred across into the mandatory fields

Confirm details and click OK
- Based on the information entered, the Account Holder will then receive account creation instructions from the My Health Memory team
If the Account Holder does not have an established relationship to the patient:

Select the Account Holder's Relationship to person

Enter a Family Name, First Name, Mobile number and Email Address

Confirm details and click OK
- Based on the information entered, the Account Holder will then receive account creation instructions from the My Health Memory team
To create multiple additional My Health Memory account holders for the patient:

Click on the My Health Memory Tab
-
Select No to Opt Out of My Health Memory

Choose the Account Holder's Relationship with the patient

Click Yes to Modify Person Details
Note: This will remove all information related to the previously entered account holder, however, it does not delete their account.

Enter a Family Name, First Name, Mobile number and Email Address

Click OK
If the patient is over 14 years old, they have the right to request their own account

Click on the My Health Memory Tab

Select No to Opt Out of My Health Memory

Update the Account Holder's Relationship to Self

Enter a Family Name, First Name, Mobile number and Email Address
If a patient or initial account holder requests to opt out

Go to the My Health Memory tab

Select Yes in the Opt Out of My Health Memory
