Charting Medications approved for Parent/Self Administration
As per the SCHN Guideline: Medication Administration by Parents and Carers or Self Medication by a Young Person, some medications are approved for unwitnessed administration by parent/carers or self administration by patients.
These medications have an additional drop-down box to facilitate correct documentation.
- Open MAR task for medication from List of Parent, Carer or Young Person Administered Medications:
- Confirm time of dose administration with Parent/Carer or patient
- Update Performed date/time if required
- Select appropriate option from Parent/Self Administered drop-down menu
- Sign for dose using the green tick icon
Charting Medications NOT approved for Parent/Self Administration
For medications that are not covered by the SCHN Guideline but have been reported as administered by a parent/carer or patient and have not been witnessed or checked by a nurse, the dose should still be recorded as given on the MAR to ensure the medication documentation is accurate and the safety features of eMM can be used.
If this situation occurs, often 2 nurses will be required to sign for the dose. If the nurses have not personally witnessed the administration, they need to document in the comments what the parent/carer/patient reports has happened.
Clinical staff must ensure that the parent/carer/patient understand that all further medications will be administered by nursing staff according to the MAR whilst the patient is admitted to SCHN to avoid any medication errors.
