Rescheduling Medications on the MAR - Bear Cottage Training Guide

Overview

There are two types of rescheduling:

  • Rescheduling an entire regimen
  • Rescheduling a single dose

When rescheduling, the system does not account for a previous administration, so you have to make sure that the administrations are not too close together or too far apart.

Rescheduling All Medication Times for a Medication

The mother advises that the child is used to medication at: 0700, 1400 and 1700.  You need to reschedule the medications.

  • Start by rescheduling Baclofen 25mg BD.
  • Navigate to Scheduled Medications in the MAR
  • Review the Baclofen details in the Medications column and then the scheduled time columns
  • Right click over Baclofen 25 mg in the Medications column and select Reschedule Admin Times

  • Change the schedule and review the first dose date/time in the Reschedule Window
    • 0700 is correct. Change 1900 to 1700 in the New Schedule Field n 
    • Select 1700 as the first administration date/time. In other words, you do not want to skip a dose  a
    • Reviewing this information is critical in ensuring that the next rescheduled dose is not too close or too far from the previous dose
    • Save. Password is train

    • Refresh
      • Notice how the column for the scheduled times for Baclofen 25mg has changed from 1900 to 1700
      • The yellow triangle indicates a change to schedule
      • Double check the next administration times for the Baclofen to make sure they are correct

    • Continue to reschedule all the Scheduled Medications
      • If the Save button is not available when rescheduling, make sure you have selected the Next administration date/time and not left the Current schedule

    • Baclofen 35 mg (midday to 1400)
    • Diazepam 12 hourly to start at 1900
    • Gabapentin (TDS to 0700, 1400, 1700)
    • Insulin – leave this to before lunch
    • Pentavite – change the dose to 1400
    • Omeprazole does not need to be rescheduled because already scheduled for 0700 or 1400 which is suitable
    • Vancomycin Intermittent infusion: change the next dose to your choosing
    • PRNs (eg fentanyl) and Continuous Infusions (eg PN) float with current time so they are not rescheduled (check the yellow column)

    NOTE: only future doses will be rescheduled. If a dose is in the past (to the left of the yellow column on the MAR) the date and time will not be changed by the reschedule action. Whether all tiles move depends on the time of day that you are performing this training activity. 

     

    Reschedule a Single Dose

    The patient has a procedure tomorrow and will not be available for the 1400 medications: Baclofen 35mg, Gabapentin 900mg, Insulin, Vancomycin and Penta-Vite. If other medications fall at 1400 tomorrow, then reschedule those doses as well.

    • Reschedule all medications for 1400 tomorrow to 1300
    • Right click over the 1400 administration tile for Baclofen 35mg (not the medication name)
    • Select Reschedule This Dose

    • In the Reschedule window change the time from 1400 to 1300 and select a reason from the drop down (patient unavailable).
    • Click OK (no password required)

    • Repeat for Gabapentin, Insulin, Penta-Vite and Vancomycin
    • Refresh the screen. Notice how all the medication tiles that have been rescheduled have a 1 to indicate the dose was rescheduled

    Note!!!

    • When you reschedule a single dose the next dose stays at its original time. So now there is more or less time between scheduled doses. Review the Next Dose Date/Time and the next.
    • Remember: click the medication name to reschedule the entire schedule. Click the administration tile to reschedule a single dose

    Review the MAR Summary Again

    It is crucial to review the MAR Summary again after making any changes to medication doses and schedules.

    • Check the first dose date/time for each medication
    • Check doses
    • Check administration times
    • Review any medications already given
