Ad Hoc Charting - Bear Cottage Training Guide

AdHoc charting enables staff to enter information onto the patient record via a ‘PowerForm’ - an electronic form or assessment.

  • Open the patient record before clicking AdHoc so the system knows which record to attach the PowerForm
  • Click AdHoc icon in the toolbar  1
  • PowerForms are contained in folders relating to topics or department e.g. General, Inpatient, Burns
  • There are also links to specific AdHoc forms from CareCompass tasks and Document Launcher
    • The Initial Falls assessment you completed on Care Compass is an AdHoc form.

 

Chart the Nursing Height/ Weight/ Allergies form

The patient has No Known Allergies and weighs 20.2kg measured

  • Click 1    in the toolbar
  • Select the 3  folder on the left and double click 4  on the right
  • Chart Weight is measured
  • Weight: 20.2 kg

  • No Known Allergies has already be selected

  • Click Sign 7

 

Modify an AdHoc Form

The parent remembers that the patient gets a drug rash from Morphine. View the Allergies Video (see link below) and Modify the Nursing HWA form.

  • Click Form Browser in the menu.

  • Right click over the form and select Modify

  • Click + Add button

  • Allergy: Morphine
  • Category: Drug; Reaction type: Allergy
  • Sympom: Drug Rash
  • Override No Known Allergies and sign

 

Unchart an AdHoc Form

  • If the form is completed on wrong patient
  • Highlight the form in FormBrowser. Right Click > Unchart and enter the reason - wrong patient

 

FYI:

Nursing weights are measured by nursing staff. The admission weight may be estimated/other (e.g. reported by parent).

The dosing weight is entered only by Medical Officers via the Height/ Weight/Allergies (prescribing) AdHoc form. It is viewable in the banner bar of the patient chart and must be entered every 7 days or the banner bar turns yellow. This dosing weight will pull into the dosage calculator when medications are ordered based on weight.
