To record if the patient has any Advance Care Plans (excluding a Resuscitation Plan), there is a mandatory field in the Nursing Admission Assessment.
If 'Yes' is selected, the Advance Care Planning form will display.
Enter the relevant answers and click the blue circle arrow to go back to the Admission Assessment.
The answers entered into this form are visible in the Advance Care Documents tab in the patient's chart.
If this information is not known at the time of the Admission Assessment, it can be completed in the Advance Care Documentation section of the Advance Care Documents page at any time throughout the encounter.
Local processes should be followed regarding how the Advance Care Plans and other Advance Care Documents are added to the patient's eMR.
